Pau d' arco is safe for children. Its only real potential side effects is allergic reactions, which is true for any herb and most substances, and if taken in very high doses some blood thinning and diarrhea.
If they can do capsules then I recommend 2 capsules three times daily at least 20 minutes before meals.
Taking it with a sulfur source such as a garlic capsule will increase the antiseptic effects.
