Hello ! I was wondering if someone could make me a simple table. Black background, white times new roman 12 point, and with this picture across the bottom:

This is an image I made myself, so don't worry about copyright or anything. I've tried to make the table myself, but I've managed to screw it up every time I try, so hopefully someone can help me here.
