Scary stuff indeed. Similar tale from Vietnam when we had just picked up a troop on a Dustoff when the driver advised that the Huey's tail rotor pedals were jammed. Flare pots and emergency services were organised at Nui Dat for a possible running landing (which is regularly practised I might add). Turns out the right side ashtray had dropped out of its cradle under the instrument panel and fell into the pedals, causing the jam up. Fortunately we were able to clear the problem with some pushing and pulling. From then on a safety chain was added to the ash trays to prevent it happening again. Then years later smoking was banned so that got rid of the problem and the trays.
