It is just a convention on who can write Mister and who cannot. Generally it is considered that if you are a surgeon and have done exam --then you are addressed as mister. But, then I know of surgeons who have not done any UK exams but have been on the specialist register as they have qualified from a European country and they write Mister and practising as consultant in the UK.
