Wow - in my day we had 5 people on call on call on the West Midlands rotation. It was really busy with lots of trauma.

I am surprised that you feel ST2s don't need a mid grade. At that stage they are most often still in the unconscious incompetence phase of learning but think they know it all - and don't ask for advice. Quite dangerous really.

However may be I am out of touch with reality and juniors are OK on 2 tier.

Maybe my concerns over 2 tier rotas in teaching hospitals are unfounded.
Though there is at least one I know of that is 2 tier- and likely others will follow suit.

But if you are ok with 2 tiers I will stop being concerned
