Why on earth to be trained and eat a small piece of the cake?

Consultant + NTN + fellowship + lots of s*** throughout career = Salary 75K+ "for newbie chaps"
Locum consultant (no NTN or other rubbish) = Salary 200K+

Ophthalmology services in the NHS are dying and will need a complete "refurbishment".
