There is usually one or more internal candidates. Some posts are specifically advertised with someone in mind. However, if your CV is outstanding then it is difficult to ignore nowadays as there are laypeople on the panel.
There is generally more politics in London than elsewhere. What matters is how nice you are, not if you are extremely talented and saved your organisation hundreds of thousands of pounds or enabled more patients to access treatment to preserve their vision. This is the public sector after all and it is important to be married to another consultant or have consultant relatives. Apparently it is important not to voice any opinion to disagree with anyone more senior even if you are correct and there is a risk to patients sight.
