1. They were unprepared for just how crowded Tokyo, and Japan in general is. They are used to big crowds -- they go to WDW twice a year. But he did the sheer volume of people everywhere caught them off guard. Because of the huge number of people, catching a bus mid-route is impossible. They are always full. Getting a cab is nearly as impossible. The subway is the best bet but they too will have a crush of people.
2. Language is a problem unless you speak Japanese. They have traveled Europe a lot and while in Europe you can usually find someone who speaks English in addition to the language of that country, it was rare to find anyone in Japan who spoke English. They have no need to learn English, so they don't. He tried using translation apps, but they didn't work very well there for some reason, he told me.
3. While people there are polite and restrooms and other public venues immaculately clean, the populace in general tend to keep to themselves and are neither friendly nor forthcoming. They are not nasty unfriendly like the French, but they do not want to interact with strangers if at all possible.
Oddly enough, I visited Japan twice as a young boy in the 1960s and that was my impression of the country then. While Japan had changed so very much since then economically, aesthetically, and technologically, it would seem that it hasn't changed much culturally.
