There are only enough doctors to staff a 3 tier on call (sho, reg, consultant) in large units. 90% will be 2 tier, so st1-3 might well be the only person on call other than a consultant.

It's important to remember that we don't actually deal with that many truly urgent cases. 99% of what we see on cash could wait a day. It's fine for juniors (esp ST2+) not to have a middle grade.
